WORK HOURS: DAYS, EVENINGS, MIDNIGHTS AND ROTATING WEEKENDS

QUALIFICATIONS:
1. High School Diploma or GED
2. At least 18 years of age
3. No disqualifying events on criminal background check
4. No disqualifying events on Health Care Workers Registry
5. Pass pre-employment physical, drug and alcohol test
6. Valid Drivers License required.
BASIC FUNCTION: Provide direct care services for individuals with intellectual disabilities including assistance with activities of daily living such as hygiene, dressing, and eating. Train individuals to acquire skills necessary to enhance independence. Participate in and implement Individual Habilitation Plans.
ESSENTIAL FUNCTIONS: THIS POSITION REQUIRES THE EMPLOYEE TO BE ABLE TO SUCCESSFULLY PERFORM THE FOLLOWING:
Pass DSP classroom and OJT program approved by Illinois DHS
Understand written and oral English Language
Record observations in participants' files
Write legible, clear, and accurate observations in standard English
Able to complete Medication Program required by DHS
Lift 50 pounds without assistance; transfer residents from bed to wheelchair and back; operate mechanical lift
Stand, sit, walk, reach, grip, bend and lift as assessed by job analysis screening
Remain free of alcohol and/or illegal substance use while working; as determined by random testing
