Clinician Type: NP/PA
Unit/Dept: Family Practice/Primary Care

Unit Details: Outpatient | All Ages | No Call
Daily Patient Volume: Average of 22 to 24 patients per 8 hours, an average of 25-27 with a 9-hour shift
Additional Providers/Support Staff: MD, APP's and MA's
EMR System(s): eClinicalWorks (eCW)
Lic/Certs: Active CA lic, DEA
BC/BE: Board Certified

Credentialing Timeframe: Quick, can start in 4 weeks
Start Date: ASAP
Duration: 3-4 months w/ possibility to extend
Shift: full-time, 5 days/week (8-hour shifts) or 4 days/week (9-hour shifts). The current hours of operation do not include Saturdays; however, this can change based on the needs of the organization
Housing/Travel: All Inclusive
Pay Structure: Preset
Notes: Provide comprehensive, continuous, and coordinated FM services within a Patient Centered Health Home model, focusing on acute and chronic conditions for patients of all ages. The position requires working in a community setting with a primarily Spanish-speaking patient population, and the team includes support from Medical Assistants. |
