Job Description

Metro is looking for a motivated and passionate PER DIEM Physical Therapist to provide in-home care. If you are a Physical Therapist ready to take the next step forward in your career, we want to hear from you. Responsibilities:

  • Accept and treat patients within your designated geographic areaย 
  • Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans
  • Develop a plan of care and treatment goals in consultation with the physician, patient, and family
  • Plan, prepare and implement customized therapy programs to restore function and promote independenceย 
  • Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
  • Provide timely and accurate documentation for each visit
  • Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
  • Deliver care in compliance with agency policies and professional practice standards
Qualifications
  • Physical Therapist Program Graduate
  • Licensed Physical Therapist in the state of New Jerseyย or eligible.
  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
  • Ability to travel to patients' homes within the assigned geographic territory
