Description

This position will be traveling to see hospice patients in the Columbia Gorge service area.

Schedule : Weekdays day shift with rotating weekend coverage

The LPN/LVN works under the direction of a registered nurse to provide quality nursing care to patients in their homes. Assists with collection of assessment data and provides input to the planning of care. Implements plan of care within the scope of licensure. Performs all duties in a manner which promotes team concept and reflects the Providence mission and philosophy.

Required Qualifications:

  • Graduate of an accredited LPN/LVN school of nursing.
  • Within 30 days of hire: National Provider BLS - American Heart Association
  • Upon hire: Oregon Licensed Practical Nurse
  • Upon hire: Washington Licensed Practical Nurse
  • Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • 1 year Experience in nursing; long term care, acute care, ambulatory care or home health and hospice care.

Preferred Qualifications:

  • Experience in home health, hospice or public health.
