The CHHA or CNA provides direct care to residents while promoting residents' highest level of functioning as directed by management. Assists in maintaining residents' environment in a safe and neat manner. Promotes quality services within facility standards, State and Federal Regulations.

Responsibilities:

  1. Receives and gives report to management on resident status at the beginning and end of each shift and as needed. Notifies the management of any changes in resident physical and/or mental status.
  2. Provides personal care to residents per their individualized plan of care.
  3. Documents all pertinent information regarding care as assigned.
  4. Prepares residents for meals and assists to and from the dining room. Assists residents with meal set up and eating.
  5. Assists residents with toilet and incontinence care.
  6. Makes room checks and verifies resident location as directed by supervisor.
  7. Keeps residents' apartments neat and clean.
  8. Performs clinical procedures according to facility standards and scope of practice regulations.
  9. Performs vital signs, B/Ps, weights and other trained procedures as directed by management.
  10. Completes prevention and restorative care including, but not limited to, PROM, AROM, ambulation, transferring, bathing, grooming, and all other ADL care.
  11. Maintains a working knowledge of facility infection control procedures.
  12. Assists in turning, positioning, and repositioning of residents as outlined in the resident's service plan.
  13. Participates actively in Interdisciplinary Team Process including documentation of care delivered and resident condition as directed by supervisor as assigned. Attends care plan meetings as assigned and acts as resident advocate.
  14. Communicates effectively with the residents, families, visitors, members of the health care team, and other disciplines.
  15. Actively participates in facility's Quality Assurance Program.
  16. Maintains education development and compliance with State and Federal Regulations and current knowledge of resident care practices.
  17. Has a basic knowledge of the organization in order to answer basic questions.
  18. Possesses working knowledge of facility emergency procedures including fire and disaster drills. Follows facility policies.
  19. Maintains resident confidentiality and adheres to HIPAA policies.
  20. Performs other duties as assigned.

Required Skills and Experience:

  • High School Diploma or equivalent required.
  • Must have six months of experience in a caregiving role or similar function.
  • Experience with Microsoft Word and Excel preferred.
  • CNA or HHA required.
  • Excellent verbal and written communication skills.
  • Able to communicate verbally in person and over the phone.
