The Home Health Aide provides essential care and support to elderly patients, ensuring their comfort and well-being. This role involves monitoring vital signs, assisting with mobility, and delivering compassionate patient care. The aide works under the supervision of a Nurse Supervisor and may occasionally accompany residents to doctor's visits. Flexible shift arrangements include both full-time and part-time options.

Responsibilities

  • Accompany patients to their doctor visits, carry charts, take notes, and push wheelchairs.
  • Assist with patients' personal care needs, i.e., personal hygiene, etc.
  • Document services provided in an accurate and timely manner in accordance with company policy and personal care plan. Maintain patient confidentiality at all times.
  • Report any changes in the patientโ€™s condition or incidents to the RN Supervisor or therapist on time.
  • Carry out other duties that may be assigned
  • Assist patients with mobility and daily activities
  • Maintain clear communication with patients, families, and medical staff
  • Manage time effectively to meet patient care schedules

Required Qualifications

  • Minimum 1 year of experience in home health care
  • High school diploma or equivalent
  • Valid Home Health Aide (HHA) certification
  • Proficiency in basic patient care and mobility support
