We are hiring an After Hours Licensed Practical Nurse for a Hospice position providing end-of-life care to patients in their homes in the Cadillac, MI area.
What You'll Do:
  • Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
  • Provide after-hours support by conducting ongoing patient assessments focused on comfort, symptom management, and changes in condition, escalating clinical concerns to the RN as appropriate
  • Educate and support patients and caregivers during evenings, nights, and weekends by reinforcing end-of-life care plans, symptom relief strategies, and what to expect throughout the hospice journey
  • Ensure thorough, compliant EMR documentation
  • Work independently, but never alone
  • Travel to patient homes within assigned territory

After Hours LPN Qualifications
  • Active Licensed Practical Nurse (LPN) license in the state you work
  • Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
