Job Summary:
Desire Home Care, Inc. is seeking a compassionate and dedicated Registered Nurse (RN) to join our team as a Home Health RN. This individual will play an integral role in providing high-quality, personalized home health care services to our clients in San Bernardino County. The RN will be responsible for assessing, planning, implementing, and evaluating all aspects of nursing care for our patients in their homes.
Compensation- Intermittent:
  • RN Start of Care Oasis- High Tech: $140
  • RN Start of Care Oasis: $120-130
  • Recert/Resumption of Care: $80-100
  • Discharge: $80
  • RN Supervisory Visit/ Follow-Up Visit: $60-70
  • RN IV: + $10 VISIT
  • Non-Visit DC Oasis: $40-45

Responsibilities:
  • Conduct in-home assessments of patients' physical, emotional, and social needs
  • Develop individualized care plans based on assessment findings and collaborate with the patient, their family, and other healthcare team members
  • Provide skilled nursing care including wound care, medication management, and administration, and patient education
  • Monitor and evaluate patient's progress and adjust care plans accordingly
  • Maintain accurate and timely documentation of all patient care activities
  • Communicate effectively and regularly with patients, their families, and other healthcare team members
  • Work in compliance with all state, federal, and agency regulations and standards
  • Participate in ongoing education and training to maintain clinical skills and knowledge

Requirements:
  • Valid and current RN license in the state of California
  • Minimum of 1 year of recent experience in home health or related healthcare setting
  • Demonstrated knowledge and proficiency in nursing assessment, care planning, and implementation
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Have experience in pediatrics and/or geriatrics
  • Strong organizational and time management skills
  • Must have a valid CA driver's license and reliable transportation
  • Must have current BLS certificate
  • Physical and TB clearance valid within the year
  • Ability to complete documentation via agency EMR system

Work Schedule: Require a minimum of 5-10 visits per week
  • Self-determined
