Position Overview:

The Physical Therapist Assistant (PTA) supports the delivery of high-quality rehabilitative care under the supervision of a licensed Physical Therapist. This role focuses on implementing individualized treatment plans that improve residents’ mobility, reduce pain, enhance strength, and restore physical function. The PTA collaborates closely with the interdisciplinary care team to ensure consistent, resident-centered therapy. A strong foundation in geriatric rehabilitation, clinical precision, and a compassionate approach are essential to support optimal outcomes and promote residents’ independence and quality of life.

Key Responsibilities:

Clinical Support and Treatment Implementation

  • Deliver physical therapy interventions as outlined in the treatment plan developed by the supervising Physical Therapist.

  • Assist residents with therapeutic exercises, gait training, and functional mobility activities to improve strength, balance, and independence.

  • Monitor resident responses during treatment sessions and communicate progress, concerns, or changes to the Physical Therapist.

  • Support resident and family education under the direction of the Physical Therapist to promote safe and effective carryover of therapy strategies.

  • Maintain a safe, supportive, and engaging therapeutic environment that encourages resident participation and comfort.

  • Participate in interdisciplinary team meetings and contribute to care planning discussions to ensure coordinated, resident-centered care.

Documentation and Compliance

  • Accurately document treatment sessions, resident performance, and observations in accordance with facility policies and regulatory standards.

  • Adhere to HIPAA, OSHA, and infection control protocols to ensure compliance and resident safety.

Collaboration and Operational Support

  • Maintain open communication with the supervising Physical Therapist, interdisciplinary care team, and resident care staff to support continuity of care.

  • Assist in maintaining therapy equipment and supplies, reporting any issues promptly to ensure a safe and efficient therapy environment.

Education:

  • Associate’s degree from an accredited Physical Therapist Assistant program

  • Current state licensure as a Physical Therapist Assistant

Experience:

  • 1–2 years of experience in a skilled nursing facility or geriatric care setting preferred

  • Familiarity with PDPM and SNF therapy documentation standards is a plus
