Schedule
Day shift, 40 hours per week, Monday through Friday with weekend rotation.
Job Description
The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs and physician direction. Responsibilities include delegation and direction of all discipline-specific therapy services for these patients, ensuring functional treatment programs designed for an effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves while adhering to the organization's Mission Statement and Core Values.
Core Responsibilities
  • Completes comprehensive evaluations and develops appropriate treatment plans focused on patient progress towards discharge.
  • Collaborates with interdisciplinary teams to address barriers to discharge.
  • Manages individual, concurrent, and group treatment sessions based on patient care needs.
  • Ensures care is provided under signed, valid physician orders and within insurance approvals.
  • Maintains clear, concise, and timely medical record documentation.
  • Participates in the student program, providing a positive learning environment.
  • Actively pursues professional growth through education and involvement in professional activities.
  • Adheres to discipline-specific State Practice Act and maintains professional licensure in good standing.
Required Experience / Certifications / Licensure
  • BLS Certification required prior to the first shift.
  • Valid, current State license required.
  • Minimum of 2 years of experience in physical therapy.
