Avanta Care is a dedicated home care and therapy service provider committed to delivering compassionate, high-quality care to our clients. We're seeking a licensed Physical Therapist (PT) to perform evaluations only in the Barnesville, GA area.
Position Overview
The Physical Therapist will be responsible for conducting thorough initial evaluations of patients, developing individualized treatment plans, and documenting findings accurately. This is an evaluation-only role; no ongoing treatment sessions are required.
Key Responsibilities:
  • Conduct comprehensive physical therapy evaluations for patients in their homes or designated facilities.
  • Develop individualized care and treatment plans based on evaluation findings.
  • Communicate recommendations and care plans clearly to patients, families, and care teams.
  • Complete and submit timely and accurate documentation in accordance with state and agency requirements.

Qualifications:
  • Active and unrestricted Physical Therapist (PT) license in the state of Georgia.
  • Previous experience in home health or outpatient settings preferred.
