Overview

Our team of doctors, advanced care providers, nurses, therapists, and researchers provides the region's most advanced medicine at multiple facilities, campuses, clinics, and classrooms. We offer patients convenient access to innovative treatments and advancements that improve the health and overall well-being of our community.
Responsibilities
  • Conducting comprehensive needs assessments with patients and families to identify their strengths, challenges, and goals.
  • Developing collaborative case plans that outline specific interventions and support services to address identified needs.
  • Providing direct services to patients, such as individual counseling or participation in support groups.
  • Collaborating with social workers to identify and facilitate referrals to essential community resources (financial assistance, housing support, job training).
  • Working collaboratively with the social worker to address any challenges or obstacles that may arise during the case management process.
  • Participating in team meetings and case staffing discussions to ensure coordinated care and service delivery.
  • Maintaining accurate and detailed electronic medical records (EMR) documentation.
  • Completes all mandatory department, educational and hospital requirements
  • Adheres to current Infection Control and Safety Standards
  • Regular and prompt attendance
  • Ability to work schedule as defined and overtime as required
  • Related duties as assigned

Additional Information
Employees must be in a regular position, working 20 hours or more per week (.50 FTE or greater) to qualify for benefits.
Qualifications
  • High School Diploma or equivalent and 1 year of experience working with at risk youth or individuals recovering from trauma, and experience providing individual/group counseling or related interventions Required
  • Bachelor's Degree in social work, psychology, counseling, or a related field Preferred
  • Comparable combination of education and experience may substitute for the above requirements.
