Valley's EPIC residential program provides individuals with the support, structure, and resources needed to build a foundation for lasting recovery and independence. As a Residential Case Manager, you'll help clients navigate services, achieve personal goals, and overcome barriers through targeted case management, psychosocial rehabilitation, and ongoing support. Your work will play an essential role in empowering clients to make meaningful progress and move toward a healthier future.

Pay: (pay is calculated based on certification status)

• Uncertified Case Managers start at $17.50 - Uncertified Case Managers will obtain certification within the first 120 days and pay will be increased
• Certified Case Manager range starts at $19.61 - Certified Case Manager pay is calculated based on years of certified experience.

Shift Differential: additional $2/hour when clocked in for 3+ hours between 3pm-11pm

$2,000 Retention Sign-on Bonus!

Schedule: Sunday - Wednesday | 2:00pm-12:00am (midnight)

Job Summary & Deliverables

The Residential Case Manager is a certified position that provides targeted case management and psychosocial rehabilitative services as indicated by the care plan to support clients in residential settings to meet their individualized goals. Certified Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Essential Functions

• Provides targeted case management services as described by the Medicaid manual within residential programs
• Supports clients in following individualized care plans and identifying case management goals
• Completes documentation on services provided within Valley's standards
• Meets productivity expectations as assigned by supervisor
• Distributes medications as necessary
• Observes and collects client urine samples as necessary
• Transports clients to appointments as necessary

Requirements

Education

• High School diploma or equivalent

Experience

• None - see Preferred Qualifications

Licenses/Certifications

• Case Manager certification (post hire as needed)
• CPR certification (post hire as needed)
• Valley de-escalation certification (post hire)
• Driving positions require a minimum age of 21 and a current driver's license

Preferred Qualifications

• One year social services or behavioral health experience
• Bachelor's degree in Social Services, Psychology, or related field
