Patient Safety Officers (PSOs) often face challenges such as overcoming staff resistance to change, identifying root causes of incidents, and maintaining a culture of transparency around reporting errors. Addressing these issues involves clear communication, ongoing staff education, and building trust to encourage open dialogue about safety concerns. PSOs also collaborate closely with clinical teams, quality assurance, and leadership to implement evidence-based safety practices. By proactively addressing these challenges, PSOs help drive continuous improvement and safer patient care environments.
