Job description

Providence PACE is seeking a full-time physician to join the Portland, Oregon team. This is an excellent opportunity for a primary care provider who enjoys variety, values team-based care and is able to float across seven PACE centers throughout the Portland metro area.
You will join an established, interdisciplinary, multi-center program that provides older adults with comprehensive whole-person care, including medical, behavioral health, therapeutic, and recreational services. The PACE program focuses on keeping older adults active and fostering independence. Some of your visits will be in outpatient primary care clinics, but many will be in private residences, assisted living facilities, adult care homes, skilled nursing facilities, and other community-based settings. Join an exceptional team of caregivers who are deeply committed to the health, dignity, and well-being of older adults.

  • Full-time at 1.0 FTE
  • Monday to Friday, daytime hours
  • Rotating coverage assignments across seven Portland metro area Providence PACE centers
  • Care provided in a variety of settings, primarily community-based but also outpatient clinics
  • Opportunity for continuity coverage by temporarily assuming a panel for up to six months
  • Collaborate closely with physicians, nurse practitioners, nurse care managers, social workers, pharmacists, psychiatric/mental health clinicians, therapists, and other interdisciplinary team members
  • Ideal candidates are resourceful, clinically confident, and able to build trust quickly with participants, families, and care teams
  • 1 year of post-graduate clinical experience working with a geriatric population preferred
  • Training and/or experience in palliative care and/or hospice valuable

Providence PACE is seeking a full-time Physician to join its team in Portland, Oregon.
