We are a trusted and growing Home Health organization dedicated to providing high-quality, compassionate care to patients in the comfort of their homes. Our team delivers personalized skilled nursing services and supports patients and families throughout the recovery and care journey.

Position Summary

We are seeking a skilled and caring Registered Nurse (RN) to join our Home Health team. You will provide in-home skilled nursing visits, patient assessments, education, and care coordination. This role requires strong clinical judgment, autonomy, and excellent communication skills.

Key Responsibilities

Complete in-home skilled nursing assessments and treatments

Administer medications and perform wound care, injections, and other skilled procedures

Collaborate with interdisciplinary team (MD, PT, OT, MSW, HHA)

Update and follow care plans per physician orders

Provide health education to patients and caregivers

Maintain accurate and timely documentation (EHR)

Communicate changes in condition promptly

Travel locally to patient homes in Covina, Glendora, Walnut areas

Qualifications

Active California RN license (required)

Home Health experience preferred but not required

Strong communication and clinical assessment skills

Reliable transportation and valid driver’s license

Ability to work independently

Knowledge of OASIS (preferred but can omit if you want)

Per Diem:

Pay per visit (SN visits, SOC, ROC, Recert, DC, etc.)
