Job Summary
We are seeking a Direct Care PRP Counselor to join our team and support youth and adults with mental illness in enhancing their quality of life. As a PRP Counselor, you will provide essential services to help clients achieve their rehabilitation goals with daily living skills, personal hygiene, organization, building supportive networks, and all while promoting a positive and supportive living environment. The ideal candidate is patient, compassionate, reliable, and committed to fostering independence and empowerment for individuals in need.
 
Responsibilities 
  • Meet with clients for individual and group sessions;
  • Participate in all treatment planning, communicate consistently with clinical staff, Program Director, and clients' families;
  • Attend and complete Ascend's required training; 
  • Maintain contact with the client’s family and treatment team; 
  • Implementation of rehabilitation activities; 
  • Assist clients in achieving their individual goals by teaching and supporting them while linking them to various community resources. 
  • Maintain quality records and timely documentation.
 
Qualifications
  • 2 years experience working with mentally ill children or adults 
  • Complete & pass a federal CJIS background check
  • Valid driver’s license and reliable transportation
  • Ability to adhere to all health and safety guidelines
  • Excellent communication and interpersonal spills
  • Ability to maintain quality records and timely documentation
