EDUCATION, EXPERIENCE, TRAINING

1.ย ย ย  3-5 years work experience in Internal Medicine or Family Practice.

2.ย ย ย  Current State Medical License for MO and KS - MD or DO

3.ย ย ย  Unrestricted Current DEA Registration

4.ย ย ย  Current State Board of Pharmacy Controlled Substance Certificate of Registration

5.ย ย ย  BLS (AHA or ARC) or ACLS (AHA or ARC) certification required
