In addition to maintaining standard operations, the Interim Director will serve as the executive sponsor and project lead for two critical growth initiatives: the expansion of the hospital's ambulatory infusion footprint and the optimization of the 340B drug pricing program, including a comprehensive Third-Party Administrator (TPA) software implementation.
Key Responsibilities
Pharmacist-in-Charge (PIC) Accountability
  • Assume full legal and operational responsibility for the hospital and retail pharmacy licenses, ensuring absolute adherence to Montana Board of Pharmacy rules, CMS conditions of participation, and DNV standards.
Daily Management
  • Oversee all pharmacy personnel, including scheduling, clinical assignments, performance management, and mentorship of clinical pharmacists and technicians.
Financial & Formulary Stewardship
  • Manage the pharmacy's operational and capital budgets. Monitor drug spend, manage inventory levels across both sites, and collaborate with the Pharmacy and Therapeutics (P&T) Committee.
Quality & Safety
  • Maintain strict adherence to USP <795>, USP <797>, and USP <800> standards across all compounding environments.
Deliverable A: 340B Program Expansion & TPA Implementation
  • TPA Software Rollout: Act as the operational lead for the implementation and configuration of the new 340B TPA software platform. Ensure proper electronic health record (EHR) data feeds, data mapping, and cross-walk validation.
  • Split-Billing & NDC Mapping: Establish and validate split-billing parameters and National Drug Code (NDC) configurations to automate compliant inventory replenishment.
  • Contract Pharmacy Management: Oversee the expansion and enrollment of new contract pharmacy agreements within the Gallatin County footprint.
  • Audit Readiness: Establish automated internal auditing procedures, continuous data validation routines, and compliance dashboards to protect against diversion and duplicate discounts in preparation for HRSA audits.
Deliverable B: Ambulatory Infusion Site Expansion
  • Regulatory Design Oversight: Partner with facilities, engineering, and compliance teams to ensure the physical expansion of the compounding cleanrooms completely aligns with USP sterile compounding requirements.
  • Clinical & Operational Workflow: Design efficient, safe clinical workflows for medication delivery from the central pharmacy to the expanded infusion chairs, optimizing nursing-pharmacy collaboration.
  • Revenue Cycle Coordination: Work closely with hospital billing and finance teams to ensure correct medical billing code (J-code) alignment, prior authorization workflows, and maximum reimbursement for the expanded service line.
Reporting & Key Performance Indicators (KPIs)
The Interim Director will report directly to the Chief Nursing Officer (CNO) and will provide monthly progress updates on the following key metrics:
  • Project milestones met for the TPA software implementation timeline.
  • Regulatory audit check-lists completed for the infusion site expansion.
  • Pharmacy labor productivity and drug spend variance relative to budget.
Required Skills
  • Proven track record of managing a 340B program, including a minimum of one full-cycle TPA software conversion/implementation.
  • Direct experience overseeing the clinical, regulatory, and physical build-out/expansion of a sterile compounding infusion center.
Education Requirements
  • Active and unencumbered Pharmacist License issued by the Montana Board of Pharmacy (or verified eligibility for immediate licensure via reciprocity).
  • Must be qualified to assume the legal responsibilities of Pharmacist-in-Charge (PIC) immediately upon start date.
