Job description

Job Title: Substitute (Life Enrichment Center)

Description:

  • Provides assistance and supervision to individuals in the least restrictive treatment model in all programmatic concerns.
  • Assists in delivering services to persons with disabilities as required and recommended in the Individual Plan (IP).
  • Participates in the development of the Individual Plan.
  • Provides personal care as needed: assistance with toileting, dining, bathing, etc.
  • Works cooperatively as a team member, to provide the best possible services to individuals.
  • Completes daily documentation of responses to training.
  • Provides training, supervision and assistance to all persons on all contracts, sub-contracts, IP training goals and community work/volunteer experiences.
  • Provides training and opportunities for each individual to exercise choice and individual rights in all aspects of their program.
  • Displays sound judgment in exercising responsibility for the safety and well being of all persons served; maintains a safe working environment at all times.
  • Provides training, supervision and support to individuals at community-based work/volunteer sites as need.
  • Position maybe responsible for driving or being an aide on a van, transporting consumers to/from Day Program. (See appropriate aide or driver job description for additional duties.)
  • Serves as an adult/peer model for normative interpersonal relations and foster an atmosphere of self-reliance, cooperation and respect.
  • Advocates for human rights in all program decisions.
  • Maintains confidentially in all matters related to the individual programs and personal circumstances.
  • Attends In-Service training opportunities as scheduled
  • Demonstrates knowledge of job duties in conscientious manner with minimal supervision under routine circumstances.
  • Performs other duties as assigned by Supervisor.

HS diploma or G.E.D.

Preference given to those with experience in the field of developmental disabilities

  1. Good verbal, written, and interpersonal skills.
  2. Good time management and organizational skills.
  3. Must be at least 19 years of age.
  4. Potentially requires a valid drivers license with a minimum of 3 years of driving experience and an agency approved driving record. Not required, but preferred.
  5. Attends and receives required training.
