The vision of the Greater Newark Health Care Coalition (GNHCC) is to ensure that all residents of Newark, East Orange, and Irvington have a fair and just opportunity to achieve their highest level of health. As one of New Jersey's four not‑for‑profit Regional Health Hubs (RHHs), GNHCC partners with clinical and social service providers to improve care coordination and health outcomes for Medicaid beneficiaries. In support of this vision and its role as an RHH, GNHCC fulfills four core functions: convening community stakeholders, collecting and analyzing data, piloting innovative programs, and advancing advocacy and policy.
Position Summary:
The Program Manager for Program Implementation & Evaluation will lead the design, execution, and assessment of innovative programs that advance GNHCC's mission. This role ensures that initiatives are implemented effectively, evaluated rigorously, and aligned with strategic priorities to improve health outcomes in the community.
Key Responsibilities:
  • Program Implementation & Evaluation (Innovative Solutions)
  • Coordinate implementation of innovative pilot programs and interventions aligned with GNHCC's strategic priorities.
  • Develop and maintain evaluation frameworks to measure program impact and outcomes.
  • Facilitate knowledge sharing and best practices across organizational pillars.
  • Ensure program activities comply with grant requirements and organizational standards.
  • Collaborate with internal teams and external partners to integrate evidence-based practices.
  • Prepare reports and presentations on program performance for leadership and stakeholders.

Qualifications:
  • Master's degree in Public Health, Social Work, Program Evaluation or Applied Research, Nonprofit Management, Community Development or related field.
  • 3+ years of experience in program management, evaluation, implementation or health related initiatives.
  • Strong analytical and project management skills.
  • Proven/significant experience with program evaluation methodologies and data-driven decision-making.
  • Excellent communication and collaboration abilities.

  • Ability to design and execute health programs and interventions from concept to completion.
  • Proficiency in developing and applying evaluation frameworks, logic models, and performance metrics.
  • Strong analytical skills for interpreting data and translating findings into actionable improvements.

Working Conditions/Physical Demand: Various community settings including home visits in Essex County; able to drive a car; use a laptop, cell phone, other office machines such as fax, copy, and scanner; able to occasionally lift 20-25 pounds.
In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $65,000.00 to USD $110,000.00. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
