Job Summary:

TheraCare Home Health is seeking a reliable and compassionate Home Health Aide (HHA) to provide high-quality personal care to patients in the home setting. In this role, you will support patients with daily living activities, promote a safe and comfortable environment, and work closely with the clinical team to ensure continuity of care. This is an ideal opportunity for someone who takes pride in patient care and wants to make a meaningful impact in the home health setting.

Qualifications

Qualifications:

  • Completion of a Certified Home Health Aide (HHA) Program
  • Minimum of 1 year of experience as a Home Health Aide in a home health agency
  • Strong understanding of personal care, patient safety, and infection control practices
  • Ability to follow an established Plan of Care under RN supervision
  • Excellent communication skills with patients, families, and clinical staff
  • Demonstrated compassion and commitment to the care of elderly and ill patients
  • Ability to accurately document care and report changes in patient condition
  • Reliable transportation with valid driver's license and current auto liability insurance
  • Current CPR certification and required health screenings
  • Ability to read, write, and communicate effectively in English
Essential Functions
  • Provide direct patient care under the supervision of a Registered Nurse in accordance with the Aide Plan of Care
  • Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting
  • Support safe patient transfers, ambulation, and positioning
  • Monitor and report changes in patient condition to the supervising nurse
  • Maintain a clean, safe, and comfortable home environment for patients
  • Assist with proper nutrition and hydration based on patient needs
  • Follow and maintain infection control and safety protocols
  • Complete accurate and timely documentation in the medical record
  • Communicate effectively with patients, families, and interdisciplinary team members
  • Participate in interdisciplinary communication and case conferences as needed
  • Maintain patient dignity, privacy, and confidentiality at all times
  • Support efficient operations through strong time management and reliability
  • Attend required in-services to maintain competency and compliance
  • Promote agency policies, procedures, and commitment to quality care
