Description
JOB SUMMARY:
To aid in providing appropriate aspects of quality patient care and support services under the direct supervision of licensed therapists. Contributes to the overall positive image of the organization of the department. Ensures that all required documentation and required forms are appropriately completed and included in patient charts.
ESSENTIAL FUNCTIONS:
  • Provides care established in the patient treatment plan under the direct supervision of a therapist and in accordance with insurance payers.
  • Communicates with patients, families, referring physicians and other members of the team.
  • Consults with supervising therapist regarding changes in treatment or condition in a timely manner.
  • Develops and enhances clinical and professional skills through knowledge and professional associations.
  • Supports and participates in department operations and development.
  • Reports any changes in patient status or needs to supervising therapist in a timely manner.
  • Ensures timely submission and delivery of all forms to billing office.
  • Ensures patient safety at all time, including safe use of equipment and gait belts, and infection control/isolation techniques.
  • Provides facilities with records as requested.
  • Ensures adequate supply of office and clinical equipment is maintained.
  • Demonstrates effective interpersonal skills and cooperative behavior with other members of the rehab team.
  • Attends team meetings and rehab training programs as directed by supervisor.
  • Participates and conducts in-services, exercise and other community related programs
  • Maintains company, patient and client confidentiality.
  • Adheres to company dress code for assigned facilities and work environment.

Position Requirements
Education Required: Minimum of High School Diploma
Licensure/Certification: Licensure or Certification as required by State guidelines. CPR Certification.
Experience: 1 year Rehabilitation Aide experience preferred.
1-2 years experience in a residential/ assisted living or skilled nursing facility preferred.
Graduates in Rehabilitation Aide program will be considered.
