Position Overview

Men's Talk Mental Health Services is seeking a dedicated and compassionate Licensed Professional Counselor (LPC) to join our team in a full-time remote role. As a crucial member of our behavioral health team, you will provide individual counseling services via telehealth platforms, enabling us to extend our reach to individuals in need of mental health support throughout Phoenix, Arizona, and beyond.

Key Responsibilities
  • Perform thorough ongoing assessments to identify the unique needs of each patient, tailoring interventions accordingly.
  • Facilitate clear and consistent communication among supervisors, direct care staff, family members, and other stakeholders to monitor and report on patient progress effectively.
  • Develop and share a tentative schedule for patient appointments, home visits, and clinical observations, ensuring consistent follow-up and accountability.
  • Conduct and coordinate home visits when needed, including in-home evaluations, chart reviews, staff interviews, and comprehensive data analysis to enhance care delivery.
  • Implement and supervise the program plan, ensuring it adheres to best practices and positively impacts patient outcomes.
  • Engage actively in Interdisciplinary Team Meetings and other relevant discussions to collaborate on patient care strategies.
  • Prepare and submit monthly and quarterly progress notes that accurately reflect individual patient developments.
  • Design and organize training sessions for staff to ensure competency in implementing the program plans and maintaining accurate documentation.
  • Provide ongoing support through follow-up interviews, home assessments, and patient observations to ensure that clients' evolving needs are met.
  • Maintain organized and up-to-date records, ensuring that all relevant information is accessible to authorized personnel.
  • Serve as a liaison with the community and the broader mental health network to aid in developmental goals and service provision.
  • Assist in the creation of procedural guidelines and training materials aimed at enhancing program effectiveness for patients.
Patient Population / Client Focus

In this role, you will have the opportunity to serve a diverse client population, including both adults and youth who may be experiencing various mental health challenges. Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
  • Current, active, and unrestricted licensure as a Professional Counselor (LPC) within the state of Arizona.
  • A minimum of two years of professional counseling experience, preferably in a clinical setting.
  • Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma-informed care is considered highly favorable.
  • Demonstrated understanding of legal and ethical standards related to mental health practices.
