Key Responsibilities
  • Conduct regular assessments to monitor patientsโ€™ progress and identify their evolving needs.
  • Facilitate effective communication with colleagues, family members, and all stakeholders involved in patient care, ensuring a holistic approach to each individual's development.
  • Develop and implement individualized treatment plans that include appointments, home visits, and strategies for progress monitoring.
  • Design and conduct home visits, integrating in-home observations, chart reviews, and staff interviews into your treatment approach.
  • Ensure the consistent and accurate implementation of program plans across all cases.
  • Participate in Interdisciplinary Team Meetings to share insights and strategies for improved patient outcomes.
  • Prepare and submit comprehensive monthly and quarterly progress reports that evaluate treatment effectiveness and recommend adjustments as needed.
  • Oversee training sessions for staff, ensuring competency in implementing program plans and utilizing essential documentation methodologies.
  • Perform follow-up assessments, home visits, and observations to continue supporting the needs of patients post-therapy.
  • Maintain detailed records to ensure transparency and accessibility of patient information for relevant parties.
  • Act as a liaison between community resources and the mental health industry to facilitate the achievement of developmental objectives for clients.
  • Contribute to the creation of training materials designed to improve treatment programs and staff competencies.
Patient Population / Client Focus

This role requires a sensitivity to diverse populations, including both children and adults, experiencing various mental health challenges. You'll engage with clients facing issues such as anxiety, depression, trauma, and substance abuse. Qualifications
  • A Masterโ€™s or Doctoral degree in Counseling, Psychology, or a closely related field is required.
