The position is fully remote, allowing counselors to work flexibly while maintaining close collaboration with a team of healthcare providers. • Conduct thorough patient intakes and assessments, focusing on diagnostic and functional evaluations to guide treatment goals.
• Collaborate with primary care providers to recognize, treat, and manage mental health and psychosocial issues that affect overall patient health.
• Provide evidence-based psychological services, including brief interventions for patients in need, tracking their progress and adjusting treatment plans accordingly.
• Initiate follow-up consultations to ensure patients are responding to interventions and to make modifications to treatment strategies as necessary.
• Develop and implement personalized relapse prevention plans for patients, contributing to their long-term stability and wellness.
• Coordinate health services delivery, ensuring seamless communication between various treatment providers with patient consent.
• Document patient progress meticulously, maintaining accurate records in compliance with agency standards.
• Engage in regular consultations with clinic management regarding behavioral health services and patient program evaluations.
• Assist patients in understanding and managing medical treatments as prescribed, including strategies for coping with medication side effects.
• Uphold the Code of Ethical Conduct for Clinical Professionals, maintaining the highest standards of care and confidentiality in all interactions.

• A Master’s Degree in Clinical Counseling or a related field is required.
• Current active LPC license in Texas.

• Familiarity with DSM-IV diagnostic criteria and varied treatment methodologies.
• Ability to evaluate individual patient needs and contribute to coordinated treatment plans.
• Strong interpersonal skills for building relationships with patients, families, and other healthcare professionals.
• Proficiency in maintaining clinical records and generating relevant progress reports.

• Bilingual candidates (English and Spanish) are preferred, as this will enhance communication with our diverse client base.
• Experience working within a Federally Qualified Health Center (FQHC) or similar environment is highly desirable.
• Knowledge of community resources and initiatives related to behavioral health.
• A demonstrated track record of fostering successful therapeutic relationships with clients.

The position offers a competitive annual salary within the range of USD 87,000 to 105,000, commensurate with experience and qualifications.
