Job description

About the Position
We are seeking a dedicated and experienced Registered Nurse (RN) to join our Home Health team. The ideal candidate will have strong experience with OASIS E2, be confident performing Start of Care (SOC) visits, and be well versed in Medicare home health guidelines.
 
Requirements
  • Active Florida RN license
  • OASIS E experience required
  • Comfortable completing Start of Care (SOC) visits
  • Solid understanding of Medicare documentation and regulations
  • Level 2 Background Screening (fingerprints) required
  • Current CPR certification
  • Reliable transportation for field visits
  • Strong communication, organization, and assessment skills
  • One year of Home Health experience 
Job Responsibilities
  • Conduct SOC, ROC, Recert, and Discharge assessments
  • Document accurately in compliance with OASIS E2 and Medicare standards
  • Deliver high quality skilled nursing care in the patient’s home
  • Communicate effectively with the interdisciplinary care team
