Job description

The Case Manager is a health care professional who assesses medical necessity and appropriateness of admissions to the hospital and placement of patients while maintaining compliance with regulatory requirements and providing quality, cost efficient patient care. This individual will also provide patients and families discharge planning in a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet the needs of each patient. This will include alternate levels of care, ie.

home care, skilled, long term care, using available community resources to promote quality, cost effective outcomes. Graduate from an approved Associate Degree; BSN or Master's program in Nursing Preferred but not required. Prior experience as a Case Manager preferred, three (3) to five (5) years of nursing experience preferred.

Current license as a registered nurse in the state of Nevada.  All other duties as assigned.
