• Manage the complete therapeutic process for a designated group of patients, ensuring a seamless coordination of care among physicians and the patient care team.
• Conduct thorough patient assessments to develop and implement personalized care plans that focus on achieving specific rehabilitation goals and enhancing patient engagement.
• Provide hands-on physical therapy treatments utilizing a variety of evidence-based techniques and procedures to support patient recovery at our facility and in home settings.
• Evaluate and monitor therapeutic and functional progress, actively participating in the adjustment of care plans as necessary to meet the evolving needs of our patients.
• Educate patients, families, and caregivers on therapeutic exercises, the use of assistive devices, and self-management strategies to ensure effective treatment beyond therapy sessions.

• A valid state license to practice Physical Therapy.
• A Doctor of Physical Therapy (DPT) degree from an accredited program.
