Position Overview

As a PRN Registered Nurse (RN), you will:

  • Provide skilled nursing care to hospice patients in their homes, facilities, or inpatient settings.
  • Perform routine, admission, and death visits in accordance with the patient’s individualized plan of care.
  • Complete comprehensive nursing assessments and communicate changes in patient condition to the appropriate members of the care team.
  • Provide pain and symptom management focused on patient comfort and quality of life.
  • Educate and support patients, families, and caregivers throughout the hospice journey.
  • Participate in the interdisciplinary team (IDT/IDG) and collaborate with physicians, CNAs, social workers, chaplains, and other care team members.
  • Coordinate services to ensure continuity and quality of patient care.
  • Complete accurate, timely, and compliant clinical documentation.
  • Follow all agency policies, infection-control standards, safety protocols, HIPAA requirements, and professional nursing standards.
  • Participate in PRN/on-call responsibilities as assigned.
  • Provide culturally responsive care that respects each patient’s values, preferences, beliefs, and individual needs.


Qualifications

  • Current, active, and unrestricted Texas Registered Nurse (RN) license.
  • Hospice experience preferred.
  • Strong clinical assessment and communication skills.
  • Ability to work independently and effectively as part of an interdisciplinary team.
  • Reliable transportation and ability to travel throughout the Houston and surrounding areas.
  • Dependable, compassionate, and professional.
  • Must successfully complete required background and exclusion screenings.
  • Must complete required hospice orientation and competency validation prior to patient assignment.
