What You'll Do

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:

  • Delivering professional nursing care across the full nursing process within your scope of practice
  • Conducting thorough patient and family assessments and contributing to individualized care plans
  • Completing all visit types as needed to meet patient and family needs
  • Administering medications and treatments per physician orders
  • Identifying nursing diagnoses from health assessment data and responding accordingly
  • Recording accurate, timely clinical notes including pain and symptom management outcomes
  • Communicating changes in patient condition to physicians and obtaining orders as required
  • Coordinating with community health partners to support seamless care transitions
  • Ensuring equipment and necessary services are arranged and available for patients
  • Maintaining and growing your clinical skills through continuing education and in-service participation
