Senior Care Solutions is a growing Medicare-certified home health agency dedicated to providing exceptional, patient-centered care throughout South Florida. We are currently seeking a compassionate and experienced PRN Registered Nurse (RN) to join our Miami team.

Position Summary

The PRN Registered Nurse is responsible for providing skilled nursing care to patients in their homes in accordance with physician orders, agency policies, and Medicare regulations. This position requires excellent clinical judgment, strong assessment skills, and timely, accurate documentation.

Responsibilities
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Evaluation, and Discharge visits.
  • Complete comprehensive patient assessments, including OASIS documentation when applicable.
  • Develop, implement, and update individualized plans of care.
  • Administer medications, treatments, wound care, injections, and other skilled nursing services.
  • Educate patients and caregivers on disease management, medications, safety, and available resources.
  • Monitor patient progress and communicate changes in condition to physicians and the interdisciplinary team.
  • Collaborate with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides to coordinate quality patient care.
  • Ensure documentation is accurate, complete, and submitted within agency timeframes.
  • Maintain compliance with Medicare Conditions of Participation, state and federal regulations, and agency policies.
  • Participate in case conferences and communicate effectively with the clinical team.
  • Travel throughout the assigned coverage area to provide patient care.
  • Current, active Florida Registered Nurse (RN) license.
  • Minimum of one (1) year of nursing experience required.
  • Home Health and Medicare experience preferred.
  • OASIS experience preferred.
  • Current BLS/CPR Certification.
  • Valid Florida driver's license, reliable transportation, and current auto insurance.
  • Excellent assessment, communication, and organizational skills.
  • Ability to work independently and manage a flexible schedule.
  • Experience with WellSky/Kinnser is a plus.
