POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

MINIMUM REQUIRED QUALIFICATIONS – 

  • FL/compact RN license
  • BLS
  • 2 years case manager experience in an acute hospital setting
  • Acute hospital experience in specialty during the past year with no more than a 90-day break

PREFERRED QUALIFICATIONS – Meditech and Midas experience; HCA experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 8:30a-5p M-F with alternating weekends

SYSTEMS – Meditech and Midas

START DATE – ASAP
