The full-time Weekend Home Health Registered Nurse delivers compassionate, high-quality nursing care to patients recovering from illness, managing chronic conditions, or navigating complex medical needs. In this role, you will assess each patient's condition, develop and implement individualized care plans, monitor progress, and adjust care as needed.

You will collaborate with physicians, therapists, caregivers, and other healthcare professionals to ensure continuity of care and prevent complications. The RN also provides patients and families with the education, clinical guidance, and emotional support needed to promote recovery, confidence, and independence.

As a Home Health RN, you will:

  • Initial and ongoing comprehensive assessments and treatment on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs.
  • Apply specific criteria for admission and re-certification to home health care to establish appropriate levels of care and the patient's eligibility.
  • Implement, develop, and document the plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.
  • Consult with and educate the patient and family regarding self-care techniques, nutrition, and dietary needs.
  • Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.
  • Initiate appropriate preventative and rehabilitative nursing procedures.
  • Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
  • Provide clinical directions to the Home Health Aide and LPN/LVN to ensure quality and continuity of service provided.
  • Prepare clinical and progress notes that demonstrate progress toward established goals.
  • Inform physician and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient and family response to care.
  • Monitor assigned cases to ensure compliance with requirements of third party payor.
  • Provide effective communication to patients, family, team members, and other health care professionals.

Requirements

  • Valid Driver's License
  • CPR Certification
  • Valid Auto Insurance
  • At least 2 years of Home Health RN experience preferred
  • HomeCare HomeBase experience preferred
