Are you a Med-Surg RN or an RN with diverse experience who wants to deliver high-acuity care—but outside of the hospital?

Open Shifts varying between (8hour Shifts): Monday - Friday 0730 - 2400 (with rotating weekends)

American River Home Care is hiring experienced Registered Nurses to provide shift-style in-home care to patients who need more than just traditional home health. Our Advance Care at Home program brings hospital-level attention into the comfort and safety of a patient's home.

This is a unique opportunity to practice at the top of your license, build real connections with patients, and help reduce hospital readmissions—all while working 1:1 in the field.


What You'll Do:
  • Provide routine and PRN nursing visits—just like on a med-surg floor, but in a home setting.
  • Deliver frequent, hands-on care (vital signs, wound care, IVs, meds, monitoring, etc.).
  • Serve as the clinical lead in managing patient care plans and communicating changes.
  • Work shift-style schedules, including every other weekend.
  • Use your critical thinking and assessment skills daily—this is not intermittent home health.
  • Build deep relationships with patients and families while improving outcomes.

What We're Looking For:
  • Active RN license in California
  • Med-Surg experience is preferred (1–2+ years)
  • Home health or hospice experience is a plus, but not required
  • Must be comfortable working independently in the field
  • Strong communication, adaptability, and problem-solving skills
  • Valid driver’s license, reliable transportation, and auto insurance
  • Willingness to cover PRN needs and work every other weekend
