Job Overview Join Elevate Care Providers LLC as a Private Duty Nurse (RN/LPN) providing one-on-one skilled nursing care in the comfort of a patient’s home. This role offers consistent hours, a low-stress environment, and the opportunity to build meaningful relationships while delivering high-quality, patient-centered care.

As a Private Duty Nurse, you will support patients with medically complex needs while promoting safety, comfort, and independence in a home setting. You’ll work closely with the patient’s care team and family to ensure continuity of care and excellent outcomes.

Responsibilities

Provide skilled one-on-one nursing care in the home setting

Administer medications as ordered

Perform G-button / tube feeding administration

Provide nebulizer treatments and suctioning

Monitor patient condition and report changes promptly

Assist with bathing, dressing, hygiene, and other ADLs as needed

Follow the physician’s plan of care and nursing orders

Maintain accurate and timely nursing documentation

Communicate effectively with families, care coordinators, and supervisors

Ensure infection control and patient safety at all times

Tracheostomy care and suctioning

Respiratory monitoring and support
