MTM Pharmacist - Help Patients Optimize Medications | Remote Options | Strong Work-Life Balance
Looking for clinical impact with better schedule flexibility? Join our growing Medication Therapy Management (MTM) team. Conduct comprehensive medication reviews, coach patients on adherence, and improve outcomes - often from home or with flexible hybrid schedules.
Key Responsibilities

  • Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
  • Identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
  • Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
  • Educate patients and caregivers on safe, effective medication use.
  • Maintain compliance with CMS MTM program requirements and quality metrics.
What You'll Bring
