Job description

Does pre-visit planning for scheduled patients.
*Communicates the patient needs and recommended treatment to the staff on their team.
*Maintains & updates the team's patient panel.
*Identifies, creates, & maintains panels of high risk sub populations for care management.
*Tracks performance measures monthly and shares with the team, staff and patients.
*Participates in ongoing quality improvement activities using the Model for Improvement.
*Contacts patients for follow up diagnostic tests, referrals, appointments, or medical follow up as
appropriate.
*Schedules nursing visits for follow up.
*Tracks and follows up on referrals.
*Provides patient education and self-management support.
*Triages patients on the team's panels who call or walk in and directs them appropriately.
*Documents observations, assessments, nursing interventions, treatments, and referrals according
to CN documentation standards.
*Provides direct nursing care back up as the team needs dictate.
Requirements
Must have a minimum of 3 years experience
Must have an Oklahoma or Compact / Multi-State RN License
Certs: CPR, BLS, and ACLS
Good written and verbal communication skills
Meet pre-employment credentialing clearances (Background Check, Drug Screen, etc.)
Ability to meet physical demands (stand; walk; sit; climb stairs, stoop, kneel, crouch, or crawl), and lift up to 50lbs
ORIENTATION SCHEDULE: Dept. Specific and Cerner Training will be provided on your first day. Department Manager will review Badging Process. (Note- Will need to take 2 forms of ID to get badge printed.)
