Job description

ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to join its rapidly expanding team. This role focuses on transforming care for the neediest senior population by providing affordable, VIP care that delivers better health outcomes. The PCP will be instrumental in building trusting relationships with patients, acting as a health coach, and leading care teams to achieve performance goals.

Practice Info

  • Accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.
  • PCP acts as a health coach, helping patients set short and long-term health goals.
  • PCP simplifies and prioritizes appropriately for patient behavior change and team performance goals.
  • Metrics regularly measured include patient admissions/thousand, appointment slots per day (18-21), CGCAHPS (patient experience), clinical gaps closures, and medical cost measures.
  • PCP will have an active role in the management of their center and will help cover for other providers.
  • PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.

Responsibilities

  • Independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
  • Takes full accountability for patient care and outcomes and appropriately seeks consultation from specialists while remaining responsible for detailed patient care.
  • Engages with the hospitalist whenever one of their patients is in the hospital.
  • Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
  • Leads their care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for office-visiting patients.
  • Engages with the transitional care team and others for patients unable to come to the office (in hospital, SNF, LTC, or homebound).
  • Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP.
  • Performs other duties as assigned and modified at manager’s discretion.

Compensation

  • $221,141 - $315,915 Salary
  • This position may also be eligible for a bonuses or commissions.
