About the Role: We are seeking a dedicated, board-certified or board-eligible Primary Care Physician or Nurse Practitioner to join our collaborative clinical team. In this role, you will provide comprehensive, high-quality longitudinal care to an established patient panel, emphasizing preventative medicine, chronic disease management, and evidence-based acute care.

Key Responsibilities:

  • Provide comprehensive outpatient primary care services, including physical exams, routine screenings, and acute/chronic disease management.

  • Formulate precise diagnostic and treatment plans, ordering and interpreting lab work, diagnostics, and imaging.

  • Collaborate closely with mid-level providers, clinical staff, and administrative leadership to optimize patient workflows and outcomes.

  • Maintain accurate, timely documentation within the Electronic Health Record (EHR) system in compliance with regulatory and billing standards.

  • Participate in quality improvement initiatives and peer review processes.

Qualifications:

  • MD or DO degree from an accredited medical school. 

  • Successful completion of an accredited Residency in Family Medicine or Internal Medicine.

  • Board Certification or Board Eligibility (BC/BE) in Family Medicine or Internal Medicine.

  • Active, unrestricted state medical license and DEA registration.

  • Strong interpersonal skills with a commitment to patient-centered care and efficient clinical operations.

Qualifications:

  • Master’s or Doctorate degree from an accredited Nurse Practitioner program (FNP or AGNP preferred).

  • Active, unrestricted Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) license.

  • National board certification (AANP or ANCC) as a Family or Adult-Gerontology Nurse Practitioner.

  • Active DEA license and state prescriptive authority.

  • Minimum of 1–2 years of outpatient primary care experience is highly preferred (passionate new graduates with extensive clinical rotations may be considered).
