POSITION SUMMARY

Work with assigned RN Case Manager in a supportive role to promote efficient and effective management of patient care within the LPN scope of nursing practice. Assist in completion of tasks required to ensure accuracy and timeliness of clinical documentation and coordination of care within the nursing team. Licensing required.

Essential Functions Statement(s)

  • Assist Case Manager with phone calls, comm notes in computer, & prepare information needed for weekend staff.
  • Daily follow up with hospital for status update of our patients admitted.
  • Contact field staff with all information that affects patient’s Plan of Care.
  • Print field staff’s daily/weekly schedule and other pertinent information.
  • Daily check in with field staff for missed visits adjusting schedule as needed.
  • Fax MD required information (i.e. missed visit report, fall sheet, abnormal, etc.).
  • Prepare wound care notes as needed and supply copies for field staff.
  • Complete proper forms and complete the process as required: Infection Control Sheet if patient placed on antibiotics, forwarding to Audit; Post Fall Sheet as falls are reported, forwarding to MD and Audit.
  • Receive and process reports submitted from field staff.
  • Generate MARS for patient re-certifications and supply copy to field RN.
  • Perform 14 day phone monitoring assessment for high risk re-hospitalization patients.
  • Record med list changes and verify with MD as needed.
  • Prepare orders and comm notes and update field folders accordingly.
  • Ensure lab is completed daily and results are faxed to MD with follow up on new orders.
  • Perform other duties as assigned by management.

SKILLS & ABILITIES

Education : Associate's Degree (two year college or technical school)

Experience : One to two years related experience

Computer Skills

Demonstrated ability to use software and computer provided.

Certificates & Licenses

Valid LPN Louisiana License without restrictions.

WORK ENVIRONMENT

Primary office and occasional patient home visits.
