Job description

Position Summary: Primary role and function is managing patient access to the acute inpatient rehabilitation unit by performing prescreening and precertification of potential patient admissions. Key to this role is establishing relationships with referral sources, both internal and external, and participating in referral development activities. Adhering to unit-based admission criteria and Medicare regulations for appropriate selection, recommendation, and communication to the admitting physician. Managing the referral log with appropriate prioritization and communication of patient readiness to all stakeholders. Responsible for referral and admission data tracking and reporting by entering into eRehab. Coordinate, analyze, validate and enter the demographic, functional, and quality data necessary to process and submit the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI); work with program leadership on quality of care initiatives and programs; provide back-up leadership as requested. Participate in unit-based meetings and functions; accept lead roles as requested.
Essential Duties and Responsibilities:
  • Review pertinent medical records of patients referred to the rehabilitation unit from internal and external sources
  • Assess (initially and ongoing) patient appropriateness for acute inpatient rehabilitation in collaboration with the rehab team
  • Initiate insurance authorizations and provide updated information to third party carriers as needed
  • Gather and collate daily functional improvement data from the direct care staff
  • Assess if data is complete and valid and interact with staff regarding questions, concerns and educational opportunities
  • Assist with assigning admission impairment groups as needed
  • Track assessment, completion, encoding and transmission dates for all discharges
  • Transmit data for submission to eRehab and CMS
  • Maintain referral/admission data
  • Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits
  • Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct

Required Qualifications:
Education and Training:
  • Currently licensed as a Registered Nurse in the state of Illinois with inpatient rehabilitation experience preferred or a licensed occupational, physical or speech therapist with inpatient rehabilitation experience.

Work Shift Details:
Days, Monday through Friday 8 am to 4:30 pm.
