$45-$50/hour | PRN | Flexible Scheduling

Chandler Creek Post Acute is a 133-bed skilled nursing facility located in Greer, South Carolina. We are proud of our small-town feel, strong community connections, and commitment to providing compassionate, high-quality care to our residents.

We are seeking a motivated Physical Therapist (PT) to join our team on a PRN basis. If you're passionate about helping patients regain mobility and independence, we'd love to connect with you.

Position Summary

As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, function, and overall quality of life. You will play a key role in helping patients regain independence and achieve personalized therapy goals.

Key Responsibilities
  • Perform patient evaluations and develop individualized treatment plans
  • Deliver therapeutic interventions to improve strength, balance, and mobility
  • Monitor patient progress and adjust treatment plans as needed
  • Maintain accurate and timely documentation
  • Collaborate with interdisciplinary teams to support patient outcomes
  • Educate residents and families on therapy techniques and recovery goals
Qualifications
  • Bachelor's degree required; advanced degree preferred
  • Active, unencumbered Physical Therapist license in South Carolina
  • Experience in a long-term care or rehab setting preferred
  • Familiarity with PointClickCare (PCC) and Casamba preferred
  • Strong communication and clinical skills
  • Ability to work independently and as part of a team
