With 16 communities spanning across multiple states, Vista Senior Living specializes in Assisted Living and Memory Care services. Our dedicated team ensures a nurturing environment where seniors thrive and find joy in every moment. Eden Vista of Green Bay is community located at 289 E. St. Joseph St., Green Bay, Wisconsin 54301. This community has a unique setup and known as The Village of Eden Vista and The Court of Eden Vista. It is essentially two facilities on the same block.

Position: Resident Care Coordinator 

The Resident Care Coordinator is responsible for maintaining the resident's medical records and assuring the smooth functioning of the unit. The Resident Care Coordinator is also responsible for providing and tracking supplies to the unit.

Essential Functions:

  • Maintains a positive and professional demeanor toward residents, visitors, families and co-workers.
  • Works closely with nursing staff, physicians and residents families in order to provide the best care for the resident. This includes, but is not limited to, reporting resident concerns to the MD/family/care manager, reporting incidents to applicable parties, documenting on incidents/changes of condition, requesting orders.
  • Assist with assessments and development of ISPs for residents in coordination with the DOW.
  • Monitor assessment schedules within PCC to ensure they are up to date.
  • Assist with updating ISPs for residents as needed d/t changes in condition or increased needs.
  • Assist with reviewing care plans with residents, families, and care managers.
  • Maintains all residents’ charts and ensures they are complete and up to date.
  • Assist DOW with developing chart for residents within point click care.
  • Maintains all medical records according to state regulations as directed by the Director of Wellness. Completes incident reports as needed.
  • Checks Medication Administration Record and makes out requisitions from the lab book daily.
  • Enters orders into MAR/PCC as needed. Follows up on medications not in the facility to ensure they are ordered. (includes diabetic supplies)
  • Assist with testing of residents/employees as needed, such as COVID testing.
  • Audits charts regularly noting deficiencies and/or discrepancies.
  • Provides supervision to caregivers. Works with DOW to provide training and corrective counseling as needed.
  • Maintains confidentiality of resident information.
  • Ensure staff have access to resident records including ISPs, face sheets, ER packets, folders for appointments.
  • Monitor call light system for extended call lights times and notify DOW of concerns to follow up on.
  • Coordinate appointments and transportation for residents.
  • Send Physician orders to Physician for review monthly.
  • Performs medication cart audits, at a minimum, of monthly.
  • Attends all required training, in-service and staff meetings.
  • Follows company uniform and grooming standards.
  • Maintains a safe working environment.
  • Adheres to all policies and procedures of Eden Senior Care.

Responsibilities:

  • Coordinate and manage care services for individuals in need
  • Develop and implement care plans based on individual needs and goals
  • Collaborate with healthcare professionals, families, and other stakeholders to ensure comprehensive care
  • Monitor and evaluate the effectiveness of care plans and make necessary adjustments
  • Provide direct support to individuals, including assistance with daily activities and personal care
  • Assist with behavior management and social work interventions
  • Coordinate transportation and drive individuals to appointments or activities as needed
  • Prepare meals and ensure dietary needs are met

Experience/Qualifications:

  • Previous experience as a caregiver is required (At least 2 years experience)
  • Must be a CBRF Med passer 
  • Familiarity with systems for managing care services is a plus
  • Ability to lift and assist individuals with mobility needs
  • Strong communication and interpersonal skills
  • Excellent organizational and time management abilities
