Senior Care Manager - Adult Intensive Services
Deliver care management services administratively and clinically to an assigned caseload of clients with mental illness, substance use disorders and/or co-occurring disorders. And, given your advanced knowledge, you will provide mentorship and train new staff and work with Leadership on special projects.
Responsibilities:
  • Provides direct services and support to clients
  • Care coordination
  • Provide mentorship and training to Care Managers
  • Assists with on-boarding requirements
  • Complete projects to support program development, data collection and analysis

Requirements:
  • Three years of experience working in Human Services, particularly in mental health, substance use, or developmental disability field
  • Two years of experience as a Care Manager or comparable role
  • Valid driver's license and use of a personal vehicle

Structure:
  • Full Time
  • AFSCME Union (dues will apply)
  • Non-exempt
  • Starting at $24.10 an hour
  • We offer a competitive salary commensurate to experience
