Care Manager - Mental Health Urgent Care
Provide case management and support services to adults seeking mental health support in an alternative setting to the Emergency Department. This position offers a combination of administrative responsibilities and direct service. Great opportunity to support individuals in a community-based setting.
Responsibilities:
  • Provides direct services and support to clients; determines quality interventions, maximizing resources and fostering the clients' independence
  • Care coordination, including communication with internal and external providers and professionals in other disciplines; schedule and attend team meetings
  • Completes referrals to other services, community resources and different levels of care
  • Establishes relationships and partnerships with local resources
  • Completes agency and program documentation and reporting
  • Participates in staff meetings, group and individual supervision, case staffing, training and other program administration
  • Assist clients with crisis prevention, de-escalation and conflict management

Requirements:
  • A combination of education and relevant experience
  • Valid driver's License and use of a personal vehicle

Structure:
  • Full time (37.5 hours)
    • Schedule: 8:30 am - 4:00 pm or 10:00 am - 5:30 pm
  • Non-Exempt
  • AFSCME Union (dues will apply)
  • Hourly rate starting at $23.68
  • We offer a competitive salary commensurate to experience
