Position Overview

Pinnacle Rehab Associates is seeking a dedicated and passionate Full-Time Physical Therapist to join our team located in Hillsborough, New Jersey. Work Environment

The Physical Therapist will work onsite, primarily in the homes of patients in the Hillsborough area. Key Responsibilities

As a Physical Therapist at Pinnacle Rehab Associates, you will:

• Directly communicate with patients to assess their needs and develop personalized treatment plans focused on preventing disability, relieving pain, and restoring function.
• Oversee Physical Therapy Assistants, ensuring the effective implementation of the treatment plan and compliance with clinical protocols.
• Evaluate patients’ functional abilities using a range of diagnostic and prognostic tests, while providing recommendations for improving home environments to enhance recovery.
• Administer manual and therapeutic exercises, guiding patients through each stage of the physical rehabilitation process.
• Deliver treatments utilizing various physical agents, such as massage and traction, tailored to the specific needs and conditions of each patient.
• Evaluate, fit, and adjust prosthetic and orthotic devices, making modifications as necessary, and collaborating with physicians for optimal patient outcomes.
• Document and report on patient progress, collaborating closely with the interdisciplinary team during case conferences to ensure comprehensive care.
• Prepare and maintain detailed clinical records, including initial evaluations, care plans, progress notes, and discharge summaries.
• Instruct family members in care techniques and equipment use, empowering them to support the patient’s rehabilitation journey.
• Participate in ongoing training and professional development to maintain current knowledge of trends and best practices in physical therapy.

Patient Population / Client Focus

This role primarily focuses on home health patients, catering to a diverse population with various conditions ranging from post-surgical recovery to chronic illness management. You will work closely with individuals requiring tailored approaches to therapy, fostering lasting relationships that encourage trust and engagement in their recovery process.

Qualifications

To be considered for this position, candidates must meet the following requirements:

• Graduation from an accredited physical therapy program.
• A minimum of one year of experience as a Physical Therapist in acute care or home care settings.
• Current and valid Physical Therapy licensure in the state of New Jersey.
• Current CPR certification and a valid driver’s license with automobile insurance.
• A commitment to professional integrity and quality patient care.
• Strong problem-solving skills with a creative approach to patient treatment strategies.

Preferred Experience

While not mandatory, candidates with additional experience in geriatric care, neurological rehabilitation, or pediatrics will be given preference. Familiarity with electronic health records (EHR) software and home health agency compliance requirements is also advantageous.
