This role is situated in a home care setting, providing the opportunity to work directly in patients' homes. • Conduct in-depth evaluations of patients' physical capabilities, reviewing their medical histories and performing detailed assessments to inform treatment plans.
• Clearly communicate evaluation findings and treatment goals to patients and their families, ensuring they understand their roles in the rehabilitation process.
• Implement and adapt treatment protocols during therapy sessions based on continuous assessments and patients’ evolving needs, drawing from evidence-based practices.
• Consult with other healthcare professionals as necessary, documenting any changes in patients’ conditions that may alter the course of care.
• Complete timely discharge planning and follow-up assessments, creating robust documentation of patient interactions, including initial evaluations, daily notes, and progress summaries.
• Demonstrate effective time management skills by maintaining an organized schedule and fulfilling productivity expectations.
• Participate in community-based marketing efforts to promote Antrum’s services while collaborating on departmental goals.
• Engage actively in continuing education and training initiatives to advance professional growth and stay updated with the latest in physical therapy practices.
• Promote an excellent customer experience by being flexible and responsive to patient needs while assisting in removing barriers to access.

As a Physical Therapist at Antrum Physical Therapy, you will focus on a diverse range of patient populations, including elderly clients requiring assistance with mobility and rehabilitation after surgery or injury. You will work closely with patients and their families to establish trust and promote adherence to therapeutic interventions, ensuring that each patient’s individual needs and preferences are prioritized in their care plans.

• A Doctorate in Physical Therapy (DPT) is required.
• A valid State license to practice Physical Therapy is necessary.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required.
• While prior work experience is not mandatory, candidates with prior experience in home care settings are encouraged to apply.

• At least 2 years of experience in a physical therapy role, preferably in a home care environment.
• Experience working with diverse patient populations and conditions.
• Strong communication skills for effective interaction with patients, families, and team members.
• Proficiency in electronic health record (EHR) systems and documentation.
