Key Responsibilities
  • Conduct comprehensive evaluations of patients to identify issues affecting their physical health, mobility, and functional abilities in both home care and rehabilitation settings.
  • Develop and implement tailored treatment plans utilizing an array of therapeutic techniques, including exercise, manual therapy, and education to aid in pain relief and mobility enhancement.
  • Monitor and document patients’ progress meticulously, adjusting treatment plans as needed to ensure the achievement of optimal outcomes.
  • Utilize evidence-based practices to recommend and fit patients with assistive devices to promote their independence and enhance self-care capabilities.
  • Engage patients’ families and caregivers in the rehabilitation process by providing practical training on therapy methods and assistive devices.

  • Participate in interdisciplinary team meetings to contribute insights and recommendations that support collective patient goals.
Patient Population / Client Focus
