About Enhanced Care Management (ECM):


ECM is a Medi-Cal benefit designed to improve health outcomes and quality of life for individuals with complex needs by addressing both medical and social determinants of health. The program is built on a foundation of comprehensive, coordinated, and person-centered care.ย 


Job Summary:
Serene Health is seeking Licensed Mental Health Therapists to support our Enhanced Care Management (ECM) program in Riverside, CA. This unique opportunity allows for two pathways of engagement:ย 

  • Contractors (1099) may support ECM functions while also providing clinical services to clients.ย 
  • Full-Time (W2) employees are expected to serve as Case Managers within the ECM program and also maintain a clinical caseload.ย 

Regardless of employment type, the role emphasizes supporting Medi-Cal members with high behavioral health needs through integrated, whole-person care.ย 


Key Responsibilities:

  • Serve as part of a multidisciplinary "Care-Team" for members with Serious Mental Illness (SMI) and/or Substance Use Disorders (SUD).ย 
  • Participate in Multi-Disciplinary Team (MDT) meetings and contribute behavioral health-informed insights.ย 
  • Join Lead Care Managers for in-person community visits to support member engagement and service coordination.ย 
  • Assist with care planning, documentation, and linkage to resources and services.ย 
  • Promote integrated care by collaborating with internal teams and external partners.ย 
  • Performs other duties as assigned.

Required Qualifications:

  • Active California license as an LCSW, LMFT, LPCC.ย 
  • Experience working with individuals with high behavioral health needs, including SMI and SUD.ย 
  • Knowledge of Medi-Cal systems and Enhanced Care Management is strongly preferred.ย 
  • Excellent communication and collaboration skills.ย 
  • Ability to work effectively in both community-based and office environments.ย 

Work Schedule:

  • In-office requirement: 1 day/week, with flexibility based on client and team needs.ย 
  • Clinical schedule (for those providing therapy) will be coordinated based on caseload and availability.ย 

Physical Requirements:ย ย 

  • Must be able to travel using personalย vehicleย to complete outreach visits. Mileage reimbursement for the use of your vehicle is at a standard rate.ย 
  • Must successfully complete andย maintainย current Tuberculosis (TB) testing and clearanceย in accordance withย company policy and applicable local/state health regulations as a condition of employment.ย 
  • Prolonged periods of sitting at an office desk on the computer.ย 
  • Lifting: Able toย lift upย to 15lbs.ย
