Geriatric Nurse Practitioner - Philadelphia, PA

Philadelphia, PA, United States

Employment Type: Permanent Full Time

Location: Philadelphia, PA

Shift: Day

Salary Range: $93,582 - $140,373 (Experience-based)

A well-known hospital in Philadelphia, PA is seeking a full-time Nurse Practitioner to drive primary medical management in alignment with PACE Organization (PO) physicians. This role involves assessments, care plans, treatment, participant education, and interdisciplinary team collaboration. Daily oversight at the PACE Organizations Clinic and rounds in contracted nursing homes are integral aspects. The position necessitates strong communication, technological proficiency, organizational skills, and adaptability within a collaborative environment.

Requirements:

  • Board certification as a Nurse Practitioner
  • 5-7 years of NP experience, preferably in long-term care, and at least 1 year with the elderly population
  • BLS, CDS Certification, DEA Registration
  • Proficiency in EMR TruChart
  • Availability for permanent placement
  • Clean/Clear malpractice record
  • Pennsylvania State License
